How to get Blood Moon Breeches in Diablo 4

If you use curse skills, you'll want these pants.

Blood Moon Breeches were one of the many unique items to undergo an overhaul in Diablo 4 Patch 1.5.0, with the pants now boasting an effect that gives Minion’s attacks a chance to randomly deal Iron Maiden or Decrepify. So, they’re perfect for Necromancers—but getting them can be challenging.

You have a chance of getting Blood Moon Breeches as a random drop by defeating enemies, participating in Legion Events, and defeating World Bosses in World Tiers Three and Four. You can also get Blood Moon Breeches as a drop from the endgame bosses Duriel, the King of Maggots, and Andariel.

RNG methods to find Blood Moon Breeches

You can randomly get these breeches simply by playing endgame content in Diablo 4. As you collect better gear in World Tier Four, you have a higher chance of getting these breeches by defeating Elites, bosses, Events, and Nightmare Dungeons.

The only problem with this method is you aren’t guaranteed to get them just because you’re defeating bosses and elites in World Tier Four. It’s purely a chance drop.

Best way to quickly farm Blood Moon Breeches

If you prefer guaranteed farming methods, take on either Duriel or Andariel; the Blood Moon Breeches are one of items these bosses can drop. It may take a few runs to get them, but you’ll get other incredible loot by target-farming these bosses. The downside is the summoning materials can be a pain to find.

To summon Duriel, you need two Mucus-Slick Eggs from Varshan and two Shards of Agony from Grigoire. To summon Andariel, you need two Sandscorched Shackles from Lord Zir and two Pincushioned Dolls from the Beast in the Ice. So, it does take some time to get these summoning materials just to take on Andariel and Duriel for drops.

After Diablo 4 Patch 1.5.0 went live on Monday, Aug. 5, Blood Moon Breeches were one of the many Ancestral Unique items that received major changes. After the update, the breeches now has the following affixes and effects:

  • Increased maximum Life.
  • Increased Overpower damage.
  • Increased Curse duration.
  • Increase to Curse Skills.
  • Your Minion’s attacks have a chance to randomly inflict Iron Maiden or Decrepify.
  • If you’re a Necromancer wearing this item, you’ll also deal increased Overpower damage to enemies afflicted by your Curses.

If you’re a Necromancer with a build that relies heavily on Curse skills, such as Decrepify or Iron Maiden, this is one of the best pants you can get in Diablo 4.
